Incoming Patient Information

HELPFUL HINTS

Allow 60-90 minutes in the clinic for your first evaluation.

Bring your insurance card/information.

For motor vehicle accidents and Labor & Industry, please bring information about your claim number, date of injury, and name of attending physician (if applicable).

Wear loose fitting clothes if possible.

Please review our extensive list of accepted insurance plans.  If your insurance
plan is not covered, or you do not have insurance, then you may pay by cash, check or credit card at the time of your visit.

Please bring any relevant records to your office visit, which may include the following:

1. Records from other treating or consulting physicians/doctors
2. Reports of lab results, MRI’s, X-rays, or other diagnostic tests
3. Actual MRI, CT-scan, or X-ray films (including hard copies or those burned onto discs).
4. Independent Medical Exam reports (IME reports)

For patients being evaluated by Dr. Schmitt, you can complete the NEW PATIENT and the PATIENT REGISTRATION forms in advance by downloading them from our website and bringing the forms to your appointment. Simply click on the FORMS link and follow the instructions.  You can also fill out these forms in the office at the time of your first appointment. Arrive early to allow enough time to fill out the necessary paperwork
